Thank you very much for taking part in this study. In the following, we are investigating what different people’s opinions are about various medical cases including decisions about resuscitation. It is not a test of medical knowledge. Please read the stories carefully and try to imagine the situation as best as you can as if you were the treating doctor. In each case, please indicate the degree to which you agree that that you as the doctor should take the action proposed.

Completing the survey should take no longer than 10 minutes. If you have already completed this survey, you should not submit a response again.

You may leave at any point. By submitting your response, you consent to the use of the data (for academic purposes only). All data will be anonymous and we cannot trace it back to you or your computer.

At the end of the survey you will be asked if you would like to enter a prize draw for two movie tickets and if so, to enter an email address. Email addresses will be stored in a separate database from answers to the survey, and we will not be able to identify individual responses.
